High Secure Services

Individuals are considered to meet the criteria for admission to DSPD high secure services if they are assessed as being more likely than not to re-offend, resulting in serious physical or psychological harm from which the victim would find it difficult or impossible to recover. The risk of re-offending must also be linked to the presence of a severe personality disorder.

Admissions to pilot services will be prioritised according to the level and imminence of risk to the public. Candidates for assessment do not need to volunteer, and work on motivation and engagement will be a key component of assessment and treatment programmes.

Most referrals are expected to be serving prisoners or those detained under mental health legislation. Admissions to DSPD hospital units must satisfy the requirements of the Mental Health Act 1983.

Structured assessments will be carried out to inform clinical decisions about whether an individual meets the DSPD criteria, and to inform (and monitor progression through) treatment. Assessments will be carried out using an agreed set of psychiatric and psychological tools.

High Secure Provision

300+ places to pilot assessment and treatment services. A planning and delivery guide for high secure service development was issued in June 2004, to help the development of these services. The capacity target is being met by developing units at:

• HMP Frankland, 80 places. The new Westgate unit has been open since May 2004

• HMP Whitemoor, 84 places. The unit situated in a converted wing of the prison has been open since 2002

• Rampton high secure hospital, 70 places. The Peaks unit opened in March 2004

• Broadmoor high secure hospital, 70 places. The Paddock Centre opened in early October 2005

Assessment and treatment is already taking place in the high secure units. Key challenges to the programme include the creation of multidisiplinary teams, developing new skills, working across organisational boundaries. We are tackling these by, in part, developing guidance that helps shape the organisation and delivery of these services, but also creating ways in which effective practice can be shared.
